TiivistelmäSubkliinisten kilpirauhasen toimintahäiriöiden eli poikkeavan tyreotropiini (TSH) -pitoisuuden ja viitealueella olevien kilpirauhashormonipitoisuuksien yleisyys lisääntyy väestön ikääntymisen myötä. TSH-pitoisuuden viitevälin yläraja suurenee ikääntyessä, mikä tulisi huomioida iäkkäiden TSH-arvoja tulkittaessa. Hyvin iäkkäiden potilaiden suurentuneiden TSH-arvojen ylihoitamista pitää varoa. Toisaalta niitä 65 vuotta täyttäneitä, joilla on merkittävä subkliininen hypertyreoosi (TSH-pitoisuus pysyvästi < 0,1 mU/l), tulisi hoitaa. Iäkkään poikkeavaa TSH-pitoisuutta tulisi arvioida kokonaisvaltaisesti niin, että potilaan muutkin yksilölliset ominaisuudet ja perussairaudet otetaan huomioon. see all
AbstractSubclinical thyroid dysfunction is common in older people. The upper limit of the reference range for thyrotropin stimulating hormone (TSH) shifts to higher concentrations in the very old, which should be taken into account when making treatment decisions. In the very old, it is thus important not to overtreat patients with high TSH concentrations and, on the other hand, remember to treat, already in somewhat younger age groups > 65 years, those characterized by persistent, significant subclinical hyperthyroidism (TSH < 0.1 mU/l). Since older patients are at risk for adverse effects of medication, the management of mildly abnormal TSH should be considered carefully, in a holistic manner. see all
