Tiivistelmä

Tämän tutkimuksen tarkoituksena oli kuvata hammaslääkäreiden kokemuksia suun terveydenhuollon julkisen ja yksityisen sektorin välisestä kumppanuudesta sekä erityisesti siihen liittyviä mahdollisuuksia ja esteitä. Sektoreiden välisen kumppanuuden vahvistaminen on yksi hallitusohjelman sosiaali- ja terveydenhuollon muutoksen painotusalue. Myös Sosiaali- ja terveysministeriön asettama ”Suun terveyttä koko väestölle 2013” -kehittämistyöryhmä vahvistaisi julkisen ja yksityisen sektorin kumppanuutta ja yhteistyötä.

Tutkimuksen kohderyhmänä olivat yksityisellä sektorilla itsenäisinä yrittäjinä ja julkisella sektorilla johtavana tai vastaavana toimivat hammaslääkärit (n=13). Aineisto kerättiin helmi-maaliskuussa 2015 teemahaastatteluilla ja analysoitiin deduktiivisella sisällönanalyysilla.

Tulokset osoittavat, että suun terveydenhuollon sektoreiden välille kaivataan enemmän kumppanuutta yhteistyötä lisäämällä. Toteutunut yhteistyö ei ole ollut tarkoituksenmukaista tai tehokasta. Kumppanuuden haasteena on ylikuormittunut julkinen sektori sekä julkisen ja yksityisen sektorin yhteyden puuttuminen. Sektoreiden välisen kumppanuuden kehittyminen ja lisääntyminen vaatii hallinnollisia ja rakenteellisia uudistuksia sekä asennemuutosta. Myös yksityisen sektorin toimijoiden roolin kasvattaminen on tärkeää, jotta yhteistyö olisi tavoitteellista, pitkäaikaista ja mielekästä. Tulevaisuudessa suun terveydenhuollon kumppanuus nähdään avoimena, arvostavana, yhteisenä tahtotilana sekä selkeänä työnjakona.

Suun terveydenhuolto tarvitsee koordinoitua palveluverkostoa sekä palvelurakenteiden ja sektoreiden välisen kumppanuuden kehittämistä vahvistaen yksityisen sektorin roolia tasavertaiseksi kumppaniksi julkiselle sektorille. Hallinnollisilla tasoilla, organisaatioiden sisällä ja julkisuudessa tulisi lisätä tietoisuutta kumppanuuden mahdollisuuksista, jotta toimintaa voidaan kehittää asiakkaiden tarpeita vastaavaksi.

The opportunities and hindrances of cross-sector partnership in oral health care from the perspective of dentists

Abstract

The purpose of this study was to describe dentists’ experiences regarding partnership between the public and private sectors in oral health care, particularly the opportunities and hindrances involved. Enhancing cross-sector partnerships is one of the focal points of the social and health care reform set down in the Government Programme. The working group “Suun terveyttä koko väestölle 2013” instituted by the Ministry of Social Affairs and Health would also like to strengthen the partnership and cooperation between the public and private sectors.

The focal group of the study consisted of dentists (n=13) working as independent entrepreneurs in the private sector, and as chief dentists or dentists in charge in the public sector. The data were gathered in February and March 2015 through focused interviews, and analysed using deductive content analysis.

The findings show that there is a wish to see more partnership between sectors in oral health care in the form of increased cooperation. The cooperation already taking place has not been efficient, nor has it fit its intended purpose. Challenges to partnership are posed by an overloaded public sector and a lack of connection between the public and private sectors. In order to develop and increase cross-sector partnership, administrative and structural renewals and changes in attitude are required. Additionally, increasing the role of actors in the private sector is important in order to achieve goal-oriented, long-term cooperation that serves its purpose. In the future, partnership in oral health care is seen as an open, appreciative, joint desired state, and as a clear-cut distribution of duties.

Oral health care requires a coordinated service network and the development of partnership between service structures and sectors, enhancing the role of the private sector as an equal partner to the public sector. Awareness of the opportunities involved in partnership should be promoted on administrative levels, within organizations and in public discourse in order to develop activities to meet the needs of the clients.
